| Procedure |

| 1. |
Fill the downloaded form, then fill the form and send with air ticket (Photocopy acceptable) and original round trip boarding passes via mail to ANA Bangkok City Office Sales Department or walk in ANA Bangkok City Office by 30 September 2008. Postage will be your own expense. |
| 2. |
Shopping coupon application will be notified and sent to address in Thailand or picked up at ANA Bangkok City Office approximately 1 month after receiving the application form. |

Gift Voucher is ANA Bangkok Special Campaign in which must be under all terms and conditions. |

Reservation and Ticketing must be made in Thailand only. |

Gift Voucher application is not transferable. |

After issuing, gift voucher cannot be redeemed for cash. |
