| 1) |
One ANA Gift Selection Application Postcard
is required for each delivery destination. |
| 2) |
Fill in only one delivery address, name and
phone number for each postcard. |
| 3) |
Choose your gift and fill in the gift number
(4 digit numeral), name of gift, quantity, and required mileage (10,000 miles
per gift). Please also note the total number of miles required.
* Maximum of four gifts can be applied per postcard. |

Make sure to fill in your membership number (10 digit
numeral), address, name, and contact phone number. |
| 5) |
Post a stamp and send it by mail. |
| 6) |
Postcard must arrive within year December 31,
2007 if your miles are expiring on December 31, 2007. |

Please allow approximately 4-6 weeks for
the delivery of items after applying as well as for some delays for reasons related
to a certain item or period. Also, please note that delivery dates for each item
may be different for applications that include multiple items. |

Changes or exchanges for items after an application has been
made due to personal reason cannot be accepted. In addition, mileage that is
of equal value to the item cannot be returned to your account or be exchanged
with other awards. |

Please note that any returns or exchanges due to a faulty image

Form" asking the food delivery date will be mailed. After receiving "Fresh
Selection Application Form", please fill in the necessary information and
