To Fill In the Hotel Mileage Retroactive Registration Form
To register correctly, please read through the notes
below.
Please do not hesitate to contact ANA Mileage Club Service Center if you have
any question.

If you forget to register mileage in advance or if there is any
difference between the statement and your actual stay, please send the form to
the applicable address for Retroactive Registration after you complete it and
attach the receipt or Visit Validation Voucher issued by the
hotel.

The address for Retroactive Registration of each hotels are different.
Please refer the details in the back page.

Retroactive registration is accepted within
6 months from the date of activity.

Documents mailed in for retroactive registration will not be
returned. Please keep copies of important documents for
your personal records.

Mileage accrual is applicable to the member only and only for
payment made directly at the hotel.
*
Stays at special group discount rates, certain other discount
rates and payment with coupons are excluded.
*
If a number of members stay together in one
room, mileage accrual will only apply to one member.
*
Mileage accrual is applicable only when the member's name and
a name in the document are the same.
